Registration will be conducted in the large gym on the north side of the school.
Please enter the school through the entrance on Highview Avenue, on the west side of the school.
Registration packets will be available at the District Office, 222 N. Kennedy Drive (Door 18) and at each
of the school offices at the end of February. Parents are encouraged to pick up a registration packet
in advance so they will have time to read and complete the required paperwork before coming
to register their child. Parents must register in person.
Students whose primary language is not English will be screened when they
come to register in order to determine class placement.
Parents need to bring the following in order to register:
 FOUR Proofs of Residency - Please bring originals and photocopies
o One of the following:
real estate tax bill, mortgage papers, signed lease, agreement of sale, OR Attestation(Affidavit)
of Residency from the resident owner where the parent and child live with the owner (affidavit
must be signed by resident)
o Three of the following (which show the proper address):
driver’s license, vehicle registration, public aid card, home/apartment insurance papers, gas bill,
electric bill, water bill, phone bill, cable bill, credit card bill OR
4 pieces of forwarded mail (4 pieces of forwarded mail count as ONE proof of residency)
 Original birth certificate and a photocopy of the birth certificate (Not the hospital certificate)
 Names and phone numbers of emergency contacts on emergency card
 Student Fee must be received to process registration materials

La inscripción tomará lugar en el gimnasio grande de la escuela. Favor de entrar la escuela por la entrada
en al oeste en la calle Highview.
Los paquetes de materiales para la inscripción serán disponibles en la oficina del distrito, 222 N. Kennedy Drive
(Puerta #18) y en todas las escuelas a finales de febrero. Pedimos que los padres recojan el paquete antes
del día de inscripción, para poder completar la información antes de inscribir a su hijo.
Los padres tienen que hacer la inscripción en persona.
Los estudiantes que hablan otro idioma (además de inglés) tendrán que tomar un breve
examen de inglés para saber cual tipo de clase le corresponde.
Los requisitos para inscribir un estudiante nuevo son:
 CUATRO pruebas de residencia - traer las foto-copias y las originales
 Uno de los siguientes:
El recibo de los impuestos de la casa, documentos de la hipoteca, contrato del
apartamento firmado, contrato de venta, o la Atestiguación (affidávit) que demuestre la
residencia del dueño del lugar donde el padre y el niño (a) viven (el affidávit debe ser
firmado por el residente)
 Tres de los siguientes (los cuales demuestren la dirección apropiada)
La licencia de conducir; el registro del vehículo; la tarjeta de ayuda pública; el seguro de la
casa/apartamento; recibos de gas, agua, eléctrico, teléfono, cable o tarjetas de crédito; o
cuatro piezas de correo enviado de la oficina de correo con la dirección corregida. (Los
cuatro cuentan por una prueba)
 Certificado original de nacimiento - traer las foto-copias y los originales (NO el de un hospital)
 Nombres y números de teléfono de contactos en caso de emergencia para la tarjeta de
emergencia.
 STUDENT FEE se tiene que pagar para poder procesar la inscripción.

Zapisy będą odbywać się w sali gimnastycznej na północnej stronie szkoły.
Wjazd do szkoły jest na ulicy Highview Avenue na zachodnej stronie szkoły.
Komplety form do zapisów będą do odebrania w District Office, 222 N. Kennedy Dr.
(wejście Nr. 18) albo w sekretariacie każdej szkoły pod koniec lutego. Zalecane jest, aby rodzice odebrali te
komplety form możliwie wcześnie co umożliwi wypełnienie i dostarczenie ich w komplecie w dniu rejestracji.
Rodzice muszą osobiście dokonać zapisu.
Uczniowie, których pierwszy język jest inny niż angielski będą przeegzaminowane w
dniu rejestracji, aby ustalić dla nich odpowiedni program nauczania.
Do zapisu potrzebne będą następujące dokumenty:
 Cztery dokumenty potwierdzające miejsce zamieszkania - orginał i kopię
o Jeden z poniżej wymienionych:
Umowa kupna domu, Rachunek podatkowy posiadłości, Umowa pożyczkowa, Umowa
wynajmowania mieszkania z podpisem właściciela, Forma (Attestation of Residency)
potwierdzająca miejsce zamieszkania rodziców I dziecka wspólnie z właścicielem domu
oraz jego podpisem
o Trzy z poniżej wymienionych z aktualnym adresem zamieszkania:
Prawo jazdy, Rejestracja samochodu, Karta public aid, Polisa ubezpieczeniowa domu,
Rachunek za gas, Rachunek za elektryczność, Rachunek za wodę, Rachunek za telefon,
Rachunek za stację kablową, Rachunek za kartę kredytową



Orginalny Akt Urodzenia z urzędu - orginal i kopię (nie ze szpitala)
Nazwisko i numer telefonu osób do skontaktowania się w razie pilnej potrzeby
Wpisowe –należy dokonać w dniu zapisu do szkoły..
