Funeral grant shall be awarded on the basis of regulations under Article 77-81 of Law dated December 17, 1998 on retirements
and pensions from Social lnsurance Fund (Dz.U[Official Gazette]2009,No 153,ltem 1227,as amended)or on the basis of Article 35,
Law of December20,1990on Farmers'Social lnsurance(Dz.U. 2008,No 50,item 291,as amended).

Funeral grant shall be awarded to personcovering costs of burial at time of death:
1) of an insured,i.e., personcoveredby retirementand pensioninsuranceon basis of Law of October13, 1998o/.rsystemof social
insurance(Dz. U. [Official Gazette]2009, No 205, Item 1585,as amended)or Farmers' SocialInsurance;
2) a personhavingan establishedright to retirementor pension;
s) an individual who, at the time of death, did not have an establishedright to retircment or pensionbut who met the conditionsfor its
acquisitionand collection;
4) a memberof the family of an individualin points 1 and 2 (in the caseof socialinsurancefor farmers- who was dependentof this
personand meeting the qualfuing conditionsfor a survivor's pension).

Amount of funeral grant shall be establishedas fol/ows;
1. If funeral costshave been provided by a memberof the family, funeral grant shallbe awardedin an amount of 4 000 PLN;
2. lf the funeral costs were paid by a person other than a member of the family, funeral grant shall be awarded in the amount of
documentedfuneral costsnot to exceed 4 000 PLN;
3. lf funeral costs were paid by more than 1 person,the funeral grant is subjectto being split among fhose persons in propoftion to the
shareof expensespaid. Each personrequestingpaymentof funeralgrantmustmake a separaterequest.

Documents required for recognitionand payment of funeral grant are:
1) Requestfor paymentoffuneral grant;
2) Abridged extract of death ceftificate;
3) Originalsof bills for burial cosfs and a copy of the statementconfirmedby the bank as a true copy if the originals were submiftedto a
bank;
4) Documents confirming blood relationship or affinity of the applicant with the deceased person- abridged extract of civil registry or
identity document;
5) Certfficateof pension insuranceregistration issuedby contributionpayer.
