1. Proszę wypełnić wszystkie pola upoważnienia (jeżeli mają zastosowanie). (please fill in all fields of
the authorization (if applicable) / bitte füllen Sie alle Felder der Zollvollmacht (wenn möglich)
2. Upoważnienie należy wydrukować w trzech egzemplarzach. (the authorization shall be print out in
three copies / die Vollmacht ist in drei Exemplaren auszudrücken)
3. Upoważnienie należy podpisać przez osobę lub osoby upoważnione do reprezentacji firmy,
zgodnie z dokumentami założycielskimi firmy (rejestrem handlowym etc.). (The authorization shall be
sign by person or persons authorized to represent the company according to the company documents (f.e. commercial
